Your Guide to Subcutaneous (SubQ) TRT Injections

Part 1: Gather Your Supplies

Before you start, wash your hands thoroughly with soap and water. Place the following items from your kit on a clean, dry surface:

Medication Vial: (e.g., Testosterone Cypionate 200mg/mL, 5mL Vial)

Syringe: (1mL Luer-Lok Syringe)

Draw Needle (Pink): (18g x 1" Needle)

Injection Needle (Orange): (25g x 5/8" Needle)

Alcohol Prep Pads (x2):

Part 2: Step-by-Step Injection Guide

Step 1: Prepare the Vial & Dose

  1. Check Medication: Confirm it is the correct medication, the dose (e.g., 0.5mL) matches your prescription, and the expiration date has not passed.
  2. Clean the Vial: Use one Alcohol Prep Pad to vigorously scrub the rubber stopper on top of the medication vial for 15 seconds. Set it aside and let it air-dry completely. Do not blow on it or wipe it. This step is critical for preventing contamination.

Step 2: Draw Your Medication

  1. Attach Draw Needle: Open the 1mL syringe and the 18g (Pink) needle. Twist the pink needle securely onto the syringe.
  2. Draw Air: Pull the plunger back to draw air into the syringe. The amount of air should be equal to your prescribed dose (e.g., if your dose is 0.5mL, pull the plunger to the 0.5mL mark).
  3. Inject Air: Insert the needle straight through the center of the rubber stopper. Push the plunger down to inject all the air into the vial.
  4. Draw Medication: Turn the vial and syringe upside down. Pull the needle tip down so it is fully in the liquid. Slowly pull the plunger back to draw slightly more than your prescribed dose.
  5. Remove Bubbles: Keeping the needle in the vial, tap the syringe gently to make any air bubbles rise to the top. Slowly push the plunger to eject the air bubbles and any excess medication back into the vial until the plunger is at your exact dose line.
  6. Remove Needle: Pull the needle straight out of the vial.

Step 3: Swap Needles for Injection

  1. Safety First: Never inject with the large 18g (Pink) needle. It is only for drawing.
  2. Cap & Remove: Carefully place the cap back on the pink needle. Twist it off the syringe and immediately place it in your sharps container.
  3. Attach Injection Needle: Open the 25g (Orange) injection needle. Twist it securely onto the syringe. Your injection is now ready.

Step 4: Administer the Injection

  1. Choose a Site: The most common site for a SubQ injection is the abdomen, at least 2 inches away from your belly button. You can also use the fatty tissue on the upper/outer thigh.
  2. Clean the Site: Use your second Alcohol Prep Pad to clean a 2-inch area of the chosen site. Let it air-dry completely.
  3. Pinch Skin: Gently pinch a 1- to 2-inch fold of skin at the cleaned site.
  4. Insert Needle: Hold the syringe like a dart at a 90-degree angle to your skin. In one quick, firm motion, insert the entire 5/8" needle into the pinched skin.
  5. Inject: Once the needle is all the way in, release the pinch of skin. Slowly and steadily push the plunger until all the medication is injected.
  6. Withdraw: Wait a moment, then pull the needle straight out.

Part 3: After Your Injection

Apply Pressure: You can apply gentle pressure to the site with a clean cotton ball or gauze. A tiny drop of blood or medication at the site is normal. Do not rub the injection site.

Dispose Safely: Immediately place the used syringe and needle (do not recap the orange needle) into your FDA-cleared sharps container.

Vial Storage: Store your medication vial at room temperature and away from direct light, as instructed on the packaging.

Frequently Asked Questions (FAQs)

Do I really need to inject air into the vial? Won't that contaminate it?

Yes, you must inject air, and no, it will not contaminate the vial if you follow the steps. Your 5mL vial is a sealed system. Injecting air equal to your dose (e.g., 0.5mL) equalizes the pressure, which allows you to easily draw your medication.

Contamination is prevented by vigorously cleaning the rubber stopper with an alcohol pad before every use and letting it air-dry. This aseptic technique is the most important step for safety.

Is this a multi-dose vial? Do I need to do anything special?

Yes. This is a multi-dose vial. To ensure safety, you must:

  1. Clean the stopper with a new alcohol pad every single time you use it.
  2. Use a new, sterile needle and syringe for every injection.
  3. The first time you open it, write today's date on the vial. Medical guidelines recommend discarding a multi-dose vial 28 days after the first puncture to prevent any risk of contamination.

What if I see a small drop of blood or medication?

This is perfectly normal and common. Simply apply gentle pressure with a clean cotton ball for a few seconds. Do not rub or massage the area.

Where should I inject?

The best site for SubQ (subcutaneous) injection is into fatty tissue. The most common and convenient site is the abdomen, at least two inches to the side of your belly button.
