After Your Order: A Step-by-Step Guide

Welcome to BioCare Fusion! We've designed our intake process to be simple and secure. This guide will walk you through the 5 key steps from your initial order to your consultation.

Step 1

Check Your Email & Register

Shortly after your purchase, you will receive an email with the subject line "Action Required: Set Up Your BioCare Fusion Patient Portal."

Action: Click the green "Register Your Patient Portal Account" button in the email. On the next screen, click the "New Patient" button to create your secure account.

Complete Your Initial Intake

After registering, you will be directed to our "Secure Intake Form." This is where you provide the initial information our clinical team needs to review your case.

Action: Fill out all required fields and upload your ID. When you are finished, you will see a "Form submitted successfully" confirmation.

Step 3

Schedule Your Appointment

After our team reviews your initial information, you will receive an email with the subject line "You have been accepted as a patient."

Action: Click the "Schedule Online" button inside this email to choose a date and time for your telehealth consultation.

Complete Your Final Intake Forms

Scheduling your appointment automatically triggers the final set of forms in your portal. You will receive an email notification that "You have a new patient form available."

Action: Click the "View Forms" button in the email. In your portal, click "Start" next to each required form to complete and sign them.

Step 5

You're All Set!

Once all forms show a "Submitted" status with a green checkmark, your intake process is complete! Your at-home lab kit will be shipped to you shortly. Please complete it as soon as it arrives. Our clinical team will have all your information ready for review before your scheduled consultation.
