1. We Create Your Account


Our team will now use the information you provided to securely create your personal patient portal account.

2. Look for Your Invitation

Within 24 hours, you will receive a separate email invitation from AdvancedMD. This email contains your unique link to register for the portal. Please keep an eye on your inbox and your spam/junk folder.

3. Complete Your Intake Forms


Once you receive the invitation, please log in and complete all your remaining intake forms. This is the final step before our clinical team can review your file.
